Tirzepatide at 40 mg/mL: the schedule in units
If the label says 40 mg / 1 mL, 80 mg / 2 mL
— it's 40 mg/mL. Read down. Every step of the standard tirzepatide ladder, as units on a U-100
insulin syringe.
Strong. Every dose fits, and the top of the ladder is only 37.5 units. The bottom is the
problem: 2.5 mg is 6.25 units, small enough that one mark's slip is a 16 % error. Reasonable from
10 mg up; awkward for the first two months.
Each step on the barrel
2.5 mg → 6.25 units on a 30-unit syringe
5 mg → 12.5 units on a 30-unit syringe
7.5 mg → 18.75 units on a 30-unit syringe
10 mg → 25 units on a 30-unit syringe
12.5 mg → 31.25 units on a 50-unit syringe
15 mg → 37.5 units on a 50-unit syringe
In-between doses at 40 mg/mL
Not on any label. They exist because vials make them possible, and they only make sense if a
prescriber put you there deliberately — usually because a full step up was too much at once.
