Semaglutide dosage chart

Same structure as the tirzepatide chart, smaller numbers, and one extra way to get it wrong: semaglutide doses are small enough that a bad concentration puts your dose down at the fiddly end of the barrel.

The schedule

Standard titration for semaglutide. Four weeks per step. Wegovy (weight) tops out at 2.4 mg; Ozempic (diabetes) at 2 mg.

2.5 mg/mL is the column with no red in it. At 1 mg/mL the top two doses overflow the syringe. At 10 mg/mL your first month is a 2.5-unit draw, which is a sliver of liquid barely off the hub. At 2.5 mg/mL every dose in the whole schedule lands somewhere you can actually see and hit. If your pharmacy lets you choose, that's the one to choose.

Semaglutide is not tirzepatide

They get talked about interchangeably and they are not interchangeable. Different molecules, different ladders, doses an order of magnitude apart. 2.4 mg is a full semaglutide dose and a rounding error of a tirzepatide one.

Which matters if you switch. People move between them fairly often — tolerance, supply, price — and the instinct to keep drawing the same number of units is exactly the wrong one. New drug, new vial, new concentration, new arithmetic. Do the whole thing again from the label.
